玉牌为和阗白玉制成，玉质凝润光洁。牌呈片状，牌头圆弧，雕饰一组对称相背的卷草龙纹，龙头间雕饰如意纹，弧线阴刻云纹为地。两面瓜型开光内以减地阳起技法琢刻纹饰。正面雕一灵鹿口衔桃枝跪卧，头顶有一蝙蝠飞舞。鹿为长角，杏仁眼，短尾，体型健壮。飞蝠圆眼丰翅。背面雕饰“福禄永昌”印章款，两侧亦饰对称蝙蝠纹，顶端装饰云纹。福、禄、寿是清代玉雕中极为流行的题材，在这块玉牌中体现的淋漓尽致。整件器物工料俱佳，牌型规矩，底面打磨平整，实为一件难得的清中期玉牌精品。
