玉牌为和阗白玉制成，玉色洁白无暇，玉质温润莹泽。牌呈长方形，上下弧圆，牌首雕饰双夔龙纹，底饰水波纹。正面开光内浅浮雕山水图，天高云淡，圆月悬空，山石霭霭，松木斜出，山间茅亭掩映，画面恬淡安适，令人心驰神往。背面双层开光内雕剔地阳文诗句：“明月松间照，清泉石上流”，出自唐代诗人王维的《山居秋暝》。此牌形制隽美别致，琢磨娴熟精湛，纹饰清雅，意境深幽，实为清代玉牌之精品。
